Medical Records


Medical records serves as a source of material for analysis, evaluation, education, research and studies of the medical care rendered by the medical, allied medical and nursing staff. It documents the integral part of a patient’s health status during their stay in this hospital.

The Medical Records Unit is located at the ground floor, left wing of the Medical Center. It is in front of the Central Sterilization Room and adjacent to Procurement Section. It is open from Monday to Friday from 8:00 am – 7:00 pm. They can be reached at 588-9900 local 250.
